Jason L. Czoschke

Jason L. “Jay” Czoschke, age 41, of Rosendale, passed away on Monday, December 8, 2014 following a hunting accident. Through organ donation, Jay is making a difference in many lives. Even in death Jay’s memory will live on through others. He was born on September 29, 1973 in Fond du Lac, the son of Larry and Mary Jo (Simon) Czoschke. He was a 1991 graduate of Laconia High School. On May 17, 2003 he married the love of his life Shawna Stephanie at St. Patrick’s Church in Fond du Lac.



Jay worked as a carpenter at C.D. Smith Construction. He was very big into hunting and just loved being outdoors and hanging out at his shop. He enjoyed going to auctions and antiquing. He was a member of Ducks Unlimited. Jay was a Green Bay Packers fan after converting from being a Bears fan! He loved being a goof and making everyone laugh.



SURVIVORS: the three most important girls in his life: his wife Shawna and their two daughters, Gabrielle and Isabelle; his parents, Larry and Mary Jo Czoschke of Eldorado; his sister, Stephanie (Davis) Tsai of Oshkosh; nephew, Ethan; his mother in law and father in law, Mike and Kathy Carlone of Fond du Lac; brother in law, Ryan Stephanie of Wausaukee; sister in law, Gina Carlone of Fond du Lac; paternal grandmother, Lucille Czoschke of Oshkosh; and many other relatives and friends. He was preceded in death by his paternal grandfather, Lawrence Czoschke; maternal grandparents, Francis and Joan Simon; one aunt, Sr. Judy Simon; one cousin, Christopher Simon; father in law, Kevin Stephanie; grandparents in law, Martin and Marie Stephanie, and Rita Fisher.



VISITATION: Friday, December 12, 2014 from 4-8 P.M. at Uecker-Witt Funeral Home, 524 N. Park Ave, Fond du Lac, WI, 54935. FUNERAL: Saturday, December 13, 2014 at 11:00 A.M. at Sacred Heart Church-Holy Family, 200 S. Peters Ave, Fond du Lac, WI, 54935. Visitation will take place on Saturday at the church from 10:00 A.M. until the time of services. Burial will follow at Rosendale Cemetery.



In lieu of flowers, memorials may be directed to Gabrielle and Isabelle’s education fund.



Jay was loved and will be missed by all that knew him…now it’s the angels turn to laugh
