Tucker Joseph Jacobitz

Tucker Joseph Jacobitz, age 4 months, of Fond du Lac, died Sunday, October 9, 2011 at Children's Hospital of Wisconsin. He was born on May 23, 2011 the son of Jason and Courtney (LaPlante) Jacobitz. He suffered from a very rare chromosomal disorder called Ring 14 Syndrome. He was preceded in death by his maternal grandfather, Robert LaPlante, and his paternal grandfather, Wayne Jacobitz.



VISITATION: Wednesday, October 12, 2011 from 10:00 A.M. until 12 noon at Holy Family Church, 271 4th St. Way, Fond du Lac. FUNERAL: Wednesday, October 12, 2011 at 12 noon at Holy Family Church with the Reverend David Funk officiating. Burial will follow at Calvary Cemetery.
